Kheir Clinic was established in 1986 to serve as a bridge between low-income, recently-immigrated, non-English speaking Korean community and local health and social service providers. Today, Kheir is a broad-based agency providing comprehensive health care for a diverse population through its 7 clinics (Federally Qualified Health Center sites), Adult Day Health Care Center and Patient Resources Department.

ASSIGNMENT SUMMARY:
The Patient Outcome Project Coordinator (POPC) supports the planning, coordination, and execution of both internally initiated and externally driven projects, while providing day-to-day administrative and organizational support to the Operations department. This role works closely with Operations leadership and cross-functional teams to keep projects organized, track timelines and deliverables, facilitate clear communication, and ensure operational priorities move forward efficiently and effectively.

The POPC is a key support role within the organization and is responsible for keeping project and operational details organized, maintaining accurate documentation, coordinating meetings and follow-ups, and helping teams stay aligned on priorities and deliverables. The POPM reports to the Associate Vice President of Operations,

KEY RESPONSIBILITIES:
Project Coordination & Support:
  • Support assigned projects with the planning, coordination, and execution from initiation through completion.
  • Maintain project timelines, task lists, status trackers, and other project documentation.
  • Coordinate project meetings, including scheduling, agendas, meeting notes, action items, and follow-up.
  • Track project deliverables, milestones, deadlines, and outstanding items to help keep projects on schedule.
  • Follow up with internal teams and stakeholders regarding assigned tasks and deadlines.
  • Help identify potential delays, gaps, or issues and escalate them to supervisor.
  • Assist with preparing project updates, status reports, presentations, and other project-related materials.
  • Maintain organized project files, documentation, and records.
  • Help coordinate communication between departments and ensure relevant information is shared with the appropriate stakeholders.
  • Support project closeout activities, including documentation, outstanding action items, and lessons learned.

Operations Support:
  • Provide administrative and coordination support to the Operations department and Operations leadership.
  • Assist with tracking operational priorities, tasks, deadlines, and initiatives.
  • Maintain operational spreadsheets, trackers, reports, and documentation.
  • Coordinate internal meetings, recurring operational check-ins, and follow-up activities.
  • Help document and maintain standard operating procedures, processes, and workflows.
  • Support the implementation and ongoing maintenance of operational processes and systems.
  • Assist with collecting, organizing, and reporting operational data and information.
  • Help coordinate cross-functional operational initiatives and ensure action items are followed through.
  • Support scheduling, logistics, vendor coordination, purchasing, or other administrative activities as needed.
  • Identify opportunities to improve organization, communication, and efficiency within day-to-day operations.

Communication & Cross-Functional Coordination:
  • Serve as a reliable point of coordination between Project Management, Operations, and other internal departments.
  • Communicate project and operational updates clearly and professionally.
  • Follow up proactively with stakeholders to keep tasks and initiatives moving.
  • Build positive working relationships across teams and contribute to a collaborative work environment.
  • Ensure information, decisions, and action items are documented and communicated appropriately.

Training and Capability:
  • Provide training and education to clinic staff on approved project-specific workflows, compliance regulations, process improvement methodologies, safety protocols, and best practices.
  • Support operational and quality-improvement initiatives when assigned as formal projects, while functional authority remains with the responsible department leader.
  • Other duties as assigned.

REQUIRED KNOWLEDGE AND COMPETENCIES:
  • Project coordination fundamentals, including supporting scope, timelines, milestones, deliverables, budgets, and risk tracking.
  • Strong oral and written communication, including preparing project updates, status reports, meeting materials, and other communications for internal stakeholders and leadership.
  • Problem-solving and issue identification, including recognizing potential risks, delays, or barriers and escalating them appropriately.
  • Critical thinking and sound judgment, with the ability to assess priorities, identify next steps, and know when to seek guidance or escalate concerns.
  • Project tracking and accountability, including maintaining action items, monitoring deliverables and deadlines, and supporting the collection and reporting of project outcomes.
  • Familiarity with clinic operations, sufficient to effectively coordinate with clinical, administrative, and operational teams.
  • Strong time-management and organizational skills, including the ability to coordinate multiple projects, priorities, and competing deadlines.
  • Cross-functional collaboration and influence, including the ability to build relationships, facilitate communication, and gain cooperation across departments without direct authority.
  • Attention to detail and follow-through, with a demonstrated ability to keep project information, documentation, and next steps organized and current.
  • Proficiency with project management and productivity tools, including spreadsheets, shared documentation, task-tracking platforms, and presentation software.

MINIMUM QUALIFICATIONS:
  • Bachelor's degree in Health Administration, Public Health, Nursing, or a related field, and 1–3 years of experience in project coordination, operations, administration, business support, or a related field.
  • Ability to work independently while knowing when to ask questions or escalate an issue.
  • A proactive, solutions-oriented approach to work.
  • Previous experience supporting projects or operational initiatives is preferred.
  • Valid California driver’s license and insurable driving record

PREFERRED QUALIFICATIONS:
  • Familiarity with community-based health care, Medicaid and Medicare, FQHCs, or public health programs.
  • Experience with EHR-based reporting and data extraction.
  • Bilingual English/Spanish or English/Korean.

WORKING CONDITIONS/PHYSICAL REQUIREMENTS:
This is primarily an office position that requires only occasional bending, reaching, stooping, lifting and moving of office materials weighing 25 pounds or less. The position requires daily use of a company provided computer and requires entering, viewing, and revising text and graphics on the computer terminal and on paper. The position requires occasional travel to attend on- and off-site meetings.
